 - add new ext_enable and ext_disable methods for selectively enabling
   or disabling only the specified subset of all supported extensions
 - add new "ust" extention for united states territories.  this
   extension is ALWAYS on unless you explicitly disable it, thus it
   retains compatibility with previous releases.
 - rewrite ext method so that the extension list is constructed in a
   more straightforward fashion.  while the behavior of the ext method
   was never fully documented, the behavior now is, for multiple flags
   for the same extension (ie, qw(usm -usm usm))), the most recent
   element in the list is the one that is honored.

 - fix specifying extension configuration at instantiation
 - added new divisions_sorted method to Locale::Geocode::Territory to
   return the divisions sorted by the specified meta

 - allow specifying the extension configuration at instantiation
 - add new extension "usps" that includes three soverign nations that
   the United States Postal Service recognizes as domestic state
   abbreviations
 - change "Micronesia" to "Federated States of Micronesia" to avoid
   confusion with the region known as Micronesia, which includes seven
   distinct territories
 - use feedback-enabled test macros (cmp_ok, is, etc) where appropriate
 - replace all occurrences of ISO-3166-1 with ISO 3166-1
