These were not used consistently and have visibility limitations, types.h - #indef _KERNEL while profile.h - publicly. Use cases like: "uintfptr_t selfpcdiff;" and "fptrdiff_t frompci;" only confuse. Given that underlying structs uprof, rawarc already use plain u_long types, there are plenty (u_long) casts elsewhere in the kernel code, follow OpenBSD and use use (u_long) casts that are clear what they do. The unused intfptr_t type does not make much sense anyway.

pg: Use long for fptrdiff_t type.

kernel: Make SMP support default (and non-optional).
The 'SMP' kernel option gets removed with this commit, so it has to be removed from everybody's configs.
