Being SW object for now, this change makes dealloc_ah() to behave like any other destroy IB flows.

Instead of RoCE drivers figuring out vlan, smac fields while working on QP/AH, provide a helper routine to read the L2 fields such as vlan_id and source mac address.
This moves logic from mlx5 driver to core for wider usage for RoCE ports.
This is a preparation patch to allow detaching netdev in subsequent patch.

RDMA: Convert drivers to use sgid_attr instead of sgid_index
The core code now ensures that all driver callbacks that receive an rdma_ah_attrs will have a sgid_attr's pointer if there is a GRH present.
Drivers can use this pointer instead of calling a query function with sgid_index. This simplifies the drivers and also avoids races where a gid_index lookup may return different data if it is changed.

When this code was reworked for IBoE support the order of assignments for the sl_tclass_flowlabel got flipped around resulting in TClass & FlowLabel being permanently set to 0 in the packet headers.
This breaks IB routers that rely on these headers, but only affects kernel users - libmlx4 does this properly for user space.

Previously, vlan id and source MAC were used from QP attributes. Since the net device is now stored in the GID attributes, they could be used instead of getting this information from the QP attributes.
IB_QP_SMAC, IB_QP_ALT_SMAC, IB_QP_VID and IB_QP_ALT_VID were removed because there is no known libibverbs that uses them.

IB/mlx4: Use correct SL on AH query under RoCE
The mlx4 IB driver implementation for ib_query_ah used a wrong offset (28 instead of 29) when link type is Ethernet. Fixed to use the correct one.

Manage RoCE gid table with logic in IB/core, which is common to all vendors, and remove the mechanism from the mlx4 IB driver. Since management of the GID cache may lead to index mismatch with the hardware GID table, a translation between indexes is required when modifying a QP or creating an address handle.

When the mlx4 IB (RoCE) device works in link aggregation mode, it exposes a single port to upper layers. Therefore, applications always set '1' in port_num attribute when modifying a QP or creating an address handle.
To make sure that a node uses all available ports the mlx4 driver will override the port_num attribute with a round robin policy.
