usb: Explicitly include correct DT includes

The DT of_device.h and of_platform.h date back to the separate
of_platform_bus_type before it as merged into the regular platform bus.
As part of that merge prepping Arm DT support 13 years ago, they
"temporarily" include each other. They also include platform_device.h
and of.h. As a result, there's a pretty much random mix of those include
files used throughout the tree. In order to detangle these headers and
replace the implicit includes with struct declarations, users need to
explicitly include the correct includes.

usb: dwc2: fix a race, don't power off/on phy for dual-role mode

When in dual role mode (dr_mode == USB_DR_MODE_OTG), platform probe
successively basically calls:
- dwc2_gadget_init()
- dwc2_hcd_init()
- dwc2_lowlevel_hw_disable() since recent change [1]
- usb_add_gadget_udc()

The PHYs (and so the clocks it may provide) shouldn't be disabled for all
SoCs, in OTG mode, as the HCD part has been initialized.

On STM32 this creates some weird race condition upon boot, when:
- initially attached as a device, to a HOST
- and there is a gadget script invoked to setup the device part.
Below issue becomes systematic, as long as the gadget script isn't
started by userland: the hardware PHYs (and so the clocks provided by the
PHYs) remains disabled.
It ends up in having an endless interrupt storm, before the watchdog
resets the platform.

The host part is also not functional, until the gadget part is configured.

The HW may only be disabled for peripheral mode (original init), e.g.
dr_mode == USB_DR_MODE_PERIPHERAL, until the gadget driver initializes.

But when in USB_DR_MODE_OTG, the HW should remain enabled, as the HCD part
is able to run, while the gadget part isn't necessarily configured.

I don't fully get the of purpose the original change, that claims disabling
the hardware is missing. It creates conditions on SOCs using the PHY
initialization to be completely non working in OTG mode. Original
change [1] should be reworked to be platform specific.

usb: dwc2: gadget: remove D+ pull-up while no vbus with usb-role-switch

When using usb-role-switch, D+ pull-up is set as soon as DTCL_SFTDISCON is
cleared, whatever the vbus valid signal state is. The pull-up should not
be set when vbus isn't present (this is determined by the drd controller).

This patch ensures that B-Session (so Peripheral role + vbus valid signal)
is valid before clearing the DCTL_SFTDISCON bit when role switch is used.
Keep original behavior when usb-role-switch isn't used.

usb: dwc2: gadget: initialize max_speed from params

DWC2 may be paired with a full-speed PHY which is not capable of
high-speed operation. Report this correctly to the gadget core by
setting max_speed from the core parameters.

Prior to commit 5324bad66f09f ("usb: dwc2: gadget: implement
udc_set_speed()") this didn't cause the hardware to be configured
incorrectly, although the speed may have been reported incorrectly. But
after that commit params.speed is updated based on a value passed in by
the gadget core which may set it to a faster speed than is supported by
the hardware. Initialising the max_speed parameter ensures the speed
passed to dwc2_gadget_set_speed() will be one supported by the hardware.

usb: dwc2: add otg_rev and otg_caps information for gadget driver

Currently the dwc2 doesn't fill in the 'otg_caps' of usb_gadget structure.
When registering a gadget device (e.g. via configfs), the
usb_otg_descriptor_init() checks the 'otg_caps' and 'otg_rev'. It defaults
to HNP and SRP bmAttributes if unspecified. There may be a mismatch with
what's being set in dwc2 params structure. This result in the descriptors
to be miss-configured in this case.

So replace 'otg_cap' bit field by 'otg_caps' structure, so hnp, srp and
otg_rev' can be configured directly in the params.
It's then provided to the gadget struct. These parameters can be tuned
for each platform. In case it's not set, it will default to current
behavior.

Also add option to setup these from the device tree by calling
of_usb_update_otg_caps(). This provides support for standard properties
such as "otg-rev", "hnp-disable" and "srp-disable" (see usb-drd.yaml).

usb: dwc2: gadget: Fix ISOC flow for BDMA and Slave

According USB spec each ISOC transaction should be performed in a
designated for that transaction interval. On bus errors or delays
in operating system scheduling of client software can result in no
packet being transferred for a (micro)frame. An error indication
should be returned as status to the client software in such a case.

Current implementation in case of missed/dropped interval send same
data in next possible interval instead of reporting missed isoc.

This fix complete requests with -ENODATA if interval elapsed.

HSOTG core in BDMA and Slave modes haven't HW support for
(micro)frames tracking, this is why SW should care about tracking
of (micro)frames. Because of that method and consider operating
system scheduling delays, added few additional checking's of elapsed
target (micro)frame:
1. Immediately before enabling EP to start transfer.
2. With any transfer completion interrupt.
3. With incomplete isoc in/out interrupt.
4. With EP disabled interrupt because of incomplete transfer.
5. With OUT token received while EP disabled interrupt (for OUT
transfers).
6. With NAK replied to IN token interrupt (for IN transfers).

As part of ISOC flow, additionally fixed 'current' and 'target' frame
calculation functions. In HS mode SOF limits provided by DSTS register
is 0x3fff, but in non HS mode this limit is 0x7ff.

Tested by internal tool which also using for dwc3 testing.

usb: dwc2: gadget: Fix sending zero length packet in DDMA mode.

Sending zero length packet in DDMA mode perform by DMA descriptor
by setting SP (short packet) flag.

For DDMA in function dwc2_hsotg_complete_in() does not need to send
zlp.

Tested by USBCV MSC tests.

usb: dwc2: gadget: Fix GOUTNAK flow for Slave mode.

Because of dwc2_hsotg_ep_stop_xfr() function uses poll
mode, first need to mask GINTSTS_GOUTNAKEFF interrupt.
In Slave mode GINTSTS_GOUTNAKEFF interrupt will be
aserted only after pop OUT NAK status packet from RxFIFO.

In dwc2_hsotg_ep_sethalt() function before setting
DCTL_SGOUTNAK need to unmask GOUTNAKEFF interrupt.

Tested by USBCV CH9 and MSC tests set in Slave, BDMA and DDMA.
All tests are passed.

usb: dwc2: Fix gadget DMA unmap direction

The dwc2 gadget support maps and unmaps DMA buffers as necessary. When
mapping and unmapping it uses the direction of the endpoint to select
the direction of the DMA transfer, but this fails for Control OUT
transfers because the unmap occurs after the endpoint direction has
been reversed for the status phase.

A possible solution would be to unmap the buffer before the direction
is changed, but a safer, less invasive fix is to remember the buffer
direction independently of the endpoint direction.

usb: dwc2: Reset DEVADDR after exiting gadget hibernation.

Initially resetting device address was done in dwc2_hsotg_irq()
interrupt handler. However, when core is hibernated USB RESET
is not handled in dwc2_hsotg_irq() handler, instead USB RESET
interrupt is handled in dwc2_handle_gpwrdn_intr() handler.

- Added reset device address to zero when core exits from gadget
hibernation.

usb: dwc2: Add device clock gating support functions

Added device clock gating support functions according
programming guide.

Moved "bus_suspended" flag to "dwc2_hsotg" struct because
we need to set that flag while entering to clock gating
in case when the driver is built in peripheral mode.

Added function names:
