Fixed printing of multiple name citations in compsurv and astro
formats

incfile: fixed 'D' so that the entire definition of a word will be
flushed when it has already been defined.
disambiguate: fixed loop conditions to avoid compares past end of list

-l flag added (set key length). 8 citation style added:
first two characters of last names of authors up to key length.

add -d flag to all programs, to change the /usr/lib/bmac directory
to whatever the user wants; default is /usr/lib/bmac

add -v flag, that forces -f (footnotes), and also places no
citation string in the output. Anticipated use is making bibliographies
that are explicitly ordered, such as in vitae (brag sheets)

When the [.$C$<fmt> citation.] format is used,
print out the citation in the currently active format. This prevents
changing the citation from affecting the entire file, which it used to
do before.

add sccs keywords per BSD conventions. Fix format strings so
