x86: add support for KVM_CAP_XSAVE2 and AMX state migration
When dynamic xfeatures (e.g. AMX) are used by the guest, the xsave area would be larger than 4KB. KVM_GET_XSAVE2 and KVM_SET_XSAVE under KVM_CAP_XSAVE2 works with a xsave buffer larger than 4KB. Always use the new ioctls under KVM_CAP_XSAVE2 when KVM supports it.

target/i386: Observe XSAVE state area offsets
Rather than relying on the X86XSaveArea structure definition directly, the routines that manipulate the XSAVE state area should observe the offsets declared in the x86_ext_save_areas array.
Currently the offsets declared in the array are derived from the structure definition, resulting in no functional change.

Include qapi/error.h exactly where needed
This cleanup makes the number of objects depending on qapi/error.h drop from 1910 (out of 4743) to 1612 in my "build everything" tree.
While there, separate #include from file comment with a blank line, and drop a useless comment on why qemu/osdep.h is included first.
