Add wcscoll_l, wcsxfrm_l, wcsncasecmp_l, wcscasecmp_l, btowc_l,
mbrlen_l, mbsinit_l, mbsrtowcs_l, wcrtomb_l, wcsrtombs_l and wctob_l.

Update old-style definitions to ANSI, remove a couple of register
definitions along the way. Fixed gcc 4.1 build (thank you vax)
