blank fourth .TH argument, and Mdocdate like everywhere else.
Use -Ios=OpenBSD for platform-independent predictable output.

Fix the obsolete .Db (toggle debug mode) macro to ignore its arguments
and not trigger an assertion when there is more than one argument;
the latter found by jsg@ with afl.
