enable qla(4) and qle(4) in ramdisks, except on sgi where the cdfs overflows

add qla and qle everywhere we currently have isp, commented out except for
qle in GENERIC on i386, amd64 and sparc64, where it's reasonably well tested
and likely to be useful.

Comment out wsmux to save kernel text, since USB support is missing from
this kernel anyway.

Replace all instances of "scsibus* at hba?" with a single "scsibus* at
scsi?" rule, similar to how ethernet PHY drivers attach at mii.
