add qla and qle everywhere we currently have isp, commented out except for qle in GENERIC on i386, amd64 and sparc64, where it's reasonably well tested and likely to be useful.

Replace all instances of "scsibus* at hba?" with a single "scsibus* at scsi?" rule, similar to how ethernet PHY drivers attach at mii.

Stop making de win over dc on alpha, and include both dc and de on all installation media now - it fits.
