objects that don't depend o

In my "build everything" tree, changing migration/vmstate.h triggers a
recompile of some 2700 out of 6600 objects (not counting tests and
objects that don't depend on qemu/osdep.h).

hw/hw.h supposedly includes it for convenience. Several other headers
include it just to get VMStateDescription. The previous commit made
that unnecessary.

Include migration/vmstate.h only where it's still needed. Touching it
now recompiles only some 1600 objects.

Move the qemu_irq and qemu_irq_handler typedefs from hw/irq.h to
qemu/typedefs.h, and then include hw/irq.h only where it's still
needed. Touching it now recompiles only some 500 objects.

The PL022 interrupt registers have bits allocated as:
0: ROR (receive overrun)
1: RT (receive timeout)
2: RX (receive FIFO half full or less)
3: TX (transmit FIFO half full or less)

A cut and paste error meant we had the wrong value for
the PL022_INT_RT constant. This bug doesn't affect device
behaviour, because we don't implement the receive timeout
feature and so never set that interrupt bit.

Change all the combinations to:

The biggest problem (apart from aesthetics) was that checkpatch complained
when we copy&pasted the code from one place to another.
